Apogeotropic Central Positional Nystagmus

Abstract

|
Positional nystagmus (PN) and vertigo are mostly the manifestations of benign paroxysmal positional vertigo (BPPV). However, PN may also occur in central pathologies, especially when the lesions are located around the fourth ventricle. We report a patient with central apogeotropic PN that was induced by lateral head turning while supine. The patient additionally showed ipsilesional spontaneous and head-shaking nystagmus with ipsilesional pursuit impairment. MRI disclosed cerebellar hemangioblastoma compressing the nodulus, uvula, and tonsil from the left side. Although apogeotropic PN is usually indicative of cupulolithiatic type of BPPV involving the horizontal semicircular canal, physicians also should be aware of central apogeotropic nystagmus.
